Executive Summary
Life Care proposes a healthcare-led community model for people living with or at risk for chronic conditions such as diabetes, high blood pressure, heart failure, overweight, obesity, and related cardiometabolic concerns.
The model brings clinical partners, care navigators, wellness professionals, community organizations, faith communities, sponsors, and technology into one coordinated pathway. It is designed to help participants understand their plans, engage consistently, address barriers, and remain connected to authorized care and support.
Life Care complements professional services. It does not independently diagnose, prescribe, change medication, or replace licensed medical or behavioral-health care.

The Life Care app concept organizes patient-generated information into responsive intelligence for authorized clinical and program review. The present concept remains a demonstration and prototype until production, privacy, security, clinical-governance, and deployment requirements are approved.
